This help article will provide some common examples of patient properties.
Patient properties are used to collect important information that doesn’t change frequently such as the patient's phone number, physician’s name/phone number, referral source, etc. Patient properties can be different kinds of questions. Here are some examples of how clinics use patient properties on Embodia:
- Name: Date of birth;
- Type: Birth date;
- Purpose: to store the patient's DOB.
- Name: Gender;
- Type: Radio button;
- Purpose: to store the patient's gender.
- Name: Phone number;
- Type: Phone number;
- Purpose: to store the patient's phone number.
- Name: Emergency contact;
- Type: Short text;
- Purpose: to store the name of the patient's emergency contact.
- Name: Emergency contact phone number;
- Type: Phone number;
- Purpose: to store the phone number of the patient's emergency contact.
- Name: Number of children;
- Type: Number;
- Purpose: to store the number of children.
- Name: Home address;
- Type: Address;
- Purpose: to store the patient's home address.
- Name: General notes;
- Type: Multi-line text;
- Purpose: to store general notes about the patient; such as hobbies, recent vacation, etc.

- Purpose: to store the name of the person who referred the patient to you.
